Annual Health & Medical Record Form (PDF)
For any and all Scouting activities, all participants must compete and turn in the Annual Health and Medical Forms – Part A and Part B (1&2). “All participants” includes parents, guardians, siblings, youth, staff, and unit leaders. Completing the Annual Health and Medical Record is the first step in making sure you have a great Scouting experience.

Scoutbook
Our Pack uses Scoutbook to track and report the progress of your scout. Once your scout is registered with BSA, you will receive an email with instructions on how to log into Scoutbook and track your scout’s progress.
Youth Protection
How much does Scouting cost?
Compared to the cost of other activities, scouting delivers a great value to its members. The basic cost includes BSA membership fees (National and Council), uniform cost, and Pack dues. However, costs can be offset by participating in our pack fundraisings.
National and Council Fees cover:
- BSA national registration
- Liability insurance
- Criminal background checks
- Youth protection
- Program resources and programs
Pack 840 Dues cover:
- Advancements and Awards
- Leader materials and training
- Den material
- Campouts/camp site fees (scout + one parent)
- Pinewood Derby registration and 1 Car Kit
- Blue and Gold Banquet
- Facility use
